Application Fee: This application has a $50 nonrefundable fee for company application or renewal and for each employee license applied for up to a maximum for a $1,000 in any calendar year.
Service Fee: All payments have a $1.50 IOWAcess fee and a $.09 epayment fee. Credit card payments have an additional credit card fee of 2.5% of the total.
