- Applicant Name Change (Marriage License or Divorce Decree)
- Maryland Professional License (does not apply to Physician, Nursing, Pharmacy or Dental Boards)
- Out-of-state Professional License
- Disciplinary Action Explanation and Supporting Documentation
- Physician Assistant Delegation Approval Letter
- US Food & Drug Administration License (only applies to Exporters, Importers and Manufacturers)
- Federal Drug Enforcement (DEA) License (only applies to Researchers Schedule I applicants)
NOTE: Establishment business name or address changes must be mailed to the Office of Controlled Substances.
