APPLICATION MUST BE COMPLETED BY THE VETERAN OR IN THE CASE OF A DECEASED VETERAN, ONLY THE AUTHORIZED DEPENDENT MAY COMPLETE THE APPLICATION.
Student must have received high school diploma, GED or equivalent; or be accepted in a dual enrollment/early college credit program PRIOR to submitting this application.
Please submit ONE of the following with the application: (1) a copy of the student’s birth certificate showing the veteran parent’s full name, (2) the student’s birth certificate with parent’s full names and a copy of the marriage license showing the veteran is married to one of the student’s parents, or (3) a copy of the completed adoption paperwork showing the student and veteran’s full names.
Student must have been accepted and plan to attend the state supported institution listed on this application. Acceptance letter must be included with application.
